Tournament registration form checklist

A good sign-up form answers every question you would otherwise text a captain about. Keep it short enough that a team finishes it on a phone, and specific enough that you can build the schedule from it.
Team details
- Team name, spelled how it should appear in the bracket
- Division
- Home city or area
- Uniform or shirt color, if you care about matchups
Contact details
- Captain name
- Mobile number for game-day changes
- A backup contact, because captains miss messages
Roster details
- Player names, and jersey numbers if your rules require them
- Number of players expected
- Coed ratio, when the division requires one
Payment details
- Entry fee for the division
- How the team is paying: online or in person
- Deposit versus full payment, if you allow deposits
- Your refund and cancellation rule, stated plainly
Scheduling details
- Any hard time restrictions, like no games before 9 a.m.
- Requests to avoid another team, when you allow requests
- Confirmation the team can play both days, for weekend events
Rules and agreement
- Confirmation the captain read the rules
- Waiver or liability agreement, if your venue requires one
- Permission to receive event updates
What to leave off
Skip anything you will not use. Long forms lose teams. Never collect card numbers in a form or a message; use a real payment flow so the card details never reach you.
Example: the short version
Team name, division, captain name, email, mobile, player count, payment method, and a rules checkbox. That is enough to seed a bracket and reach every team on game day.
